Injuries and Return to Play

Unfortunately, injuries sometimes accompany hockey, whether it is from hockey or outside of hockey. 

 To ensure players have appropriately recovered from injuries, a physician needs to confirm that each injured player is ready to return by completing the Return to Play FormThe completed form must be provided to the team trainer before the injured player can return.  At no time is the coach, parent or any individual allowed to bypass this process or the trainer.  Long term player safety is always the primary consideration.

This form will be accepted in email format also.
