Forms to be filled out for first day of hockey.
Attention Blackhawk Families,
An email was sent out late in the day on Sunday September 26, 2021:

Please see the links below for the mandatory paperwork you will need to print, complete and bring to the rink for your first ice time. The process will be busy and a bit chaotic, so we appreciate you doing your part to help us expedite the process.  We are accepting cash or cheques for the first instalment $250.00 for Point residents and $275.00 for all others ….

Again this year we will be using the same screening for players and parents (or whoever is attending with your player) as we used last year. Sportsheadz is the provider of the app. This app allows the coaching staff to see who has completed the screening. You will need to complete the screening for every practice and game! There is now an app for your mobile phone (Android and Apple).You should be getting an email by tomorrow night. The subject will be like this: "You've Been Added To The 2021-2022 U** Team."
*Team Managers will be tasked with taking attendance and checking the screening app to ensure we are in compliance for contact tracing and self screening. 

Please note: there will be also be a third party Screener checking proof of vaccination and ID at the entrance , so please have yours handy upon arrival.  We will do our very best to make sure you get through the paperwork process in an efficient manner.  We are suggesting you arrive 45 minutes ahead of your assigned ice time.  Don’t forget your patience, we will all need a healthy amount of patience but the pay off is…. A much more normal season!
