Forms to be filled out and general information.
Greetings Black Hawks families! We are mere days away from our return to the rink and first night of practice(s) of the 2022\23 hockey season!!

Please use the links below to access the mandatory paperwork you will need to print, complete, and bring to the rink for your first ice time. The process will be busy and is always a bit chaotic, so we appreciate you doing your part to help us expedite the process.

Players should arrive at the rink 1/2 hour before practice begins, allowing ample time to get dressed and meet their coaching staff. Additionally, the coach may want to have a quick pre-practice chat



*Mouthguards are mandatory whenever a player is on the ice! - This is an OMHA rule.
