Bench staff - criminal background check's
With hockey just around the corner; Coaches, Benchstaff, on ice Volunteers 18 years and older are reminded: OMHA and PEMAA require a current Criminal Reference Check.

If you had one completed for your employment, previous volunteer role etc. a copy will suffice!

As per OMHA policy and procedures, Volunteers who have submitted a Criminal Reference Check within the past three years are able to complete an affidavit stipulating there have been no new charges since their check was completed. Affidavits are available from the link below.

If you live in the City of Sarnia, you will need to contact Sarnia Police Services.

Please retain your receipts for reimbursement and reach out with any questions or concerns!
